Orientation/Continuing Education Core Content: required annually.
NOTE: Continuing education must have been completed no more than one year prior to the last day of the term you are entering. For example, to be valid for the Fall 2010 term, your CEUs must have been completed no earlier than December 20, 2009.

Legacy Health System
All students and instructors must get a photo ID from Legacy BEFORE beginning clinicals and then RETURN it after clinicals are over. Students and instructors need to go the security office at any Legacy site to obtain the photo ID.
Legacy requires that students have their Passport with them during their rotation. Students and faculty must have an up-to-date Passport that can be reviewed at any time by Legacy staff, managers, or surveyors.
All students & faculty must complete these education modules annually prior to their rotations at Legacy sites. Download this form, sign it, and return as directed by your course coordinator once you have completed all of the modules.

your name, fill in the date, and sign verifying your understanding of the
material. Turn in the signed page as directed by your course coordinator.

Glucose Meter Training: Students should attend a 30-45 minute class to allow them access to the new Abbott glucose meter during their nursing tenure at OHSU.
