Legacy Health System
All students & faculty
must complete these education modules annually prior to their rotations at
Legacy sites. Download this form,
sign it, and return it to Georgia Maudsley once you have completed all of the
modules.
Legacy requires that students have their Passport with
them during their rotation. Students and faculty must have
an up-to-date Passport that can be reviewed at any time by Legacy staff, managers,
or surveyors.

your name, fill in the date, and sign verifying your understanding of the
material. Bring
the signed page to the NLRC.
