NCLEX Testing and OSBN Licensure Information

Graduating Students:

The following information is provided to help you navigate the process for applying to take the NCLEX-RN exam and applying for licensure in the state of Oregon.  There are two different processes that should be completed simultaneously.  They are 1) OSBN application packet and 2) registering for the NCLEX-RN exam with Pearson VUE (testing agency).  Before you begin, review the eight steps of the NCLEX-RN examination process. 

To avoid delay in processing your application you must use the same name on every document you submit to OSBN and Pearson VUE.  Since you will be required to show identification at the testing site you should use the name printed on your legal identification. 

Two applications that must be completed before you take NCLEX-RN in Oregon

1. The OSBN application.
   • One part of the process involves obtaining the Dean of Nursing’s signature on a Picture Identification Form (included with the packet).  To obtain the Dean’s signature please bring your completed Picture ID form, with proper identification, to Jenifer Keltner in PH 314 for verification.  We will then forward your signed Picture ID forms directly to the board for processing.    
   • You must also submit a transcript request form (included with the packet) to the Office of Enrollment Services, Loveridge Hall.  There is no charge for this.  
2. PearsonVUE registration.
   • Linfield program code is 80-598.
   • After you register with Pearson VUE and the OSBN application process is complete and processed you will receive an Authorization to Test (ATT) from Pearson VUE.  Only then can you schedule an appointment on the Pearson VUE website.  Generally, the OSBN requires one week after the last day of the semester to process applications.

**For those students who will be applying for licensure in a state other than Oregon you must contact that state’s board of nursing for their application materials.
