Designate your tax withholdings.

Form I-9

The last page of this fillable form will provide you with a list of acceptable proof of identity.  Be sure to bring those documents with you on your first day of employment with Linfield. 

Direct Deposit 

Avoid waiting in lines at the bank!  Sign up to have your paycheck automatically deposited to your account.

Retirement Contribution Form

Turn in this form at any time to make changes to your 403(b) deferral amount, and to choose between Pre-Tax and Roth. 

Payroll Check Request Form

Use this form to request a check because of a late timecard/referral form, late contract, or if you lost your check and need a replacement.  This form is also used for a special payment, bonus, or one-time stipend.

Please note this form is only for Linfield employees, not student employees. Student employees may submit an updated Form W-4 to the payroll department to change their mailing address for tax and paystub documents.
