It must be submitted at least 10 days prior to the submission deadline.

2. If this project affects your course load and/or department curriculum, has it been approved by your department chair? Yes No Not Applicable

3. If new space, special equipment and/or technology are needed, has it been approved by Academic Affairs? Yes No Not Applicable If yes to #2 and #3, please explain

If the answer is no to questions #2 and #3, you must receive permission first before requesting approval.

All budgets should be reviewed by the director of foundation & corporate relations before submitting this form.
