RN to BSN Health Passport documents

Welcome to the Online RN – BSN degree program of Linfield College and the Linfield-Good Samaritan School of Nursing.

As you prepare to join the RN – BSN cohort with whom you will take the Linfield nursing courses, we want you to be aware of the documents that are required by the Linfield-Good Samaritan School of Nursing to be received prior to registration for nursing courses.

Before the first day of NURS 309, Transition to Professional Nursing, please complete the following:

  1. Obtain your CATNET ID.
    1. Go to http://www.linfield.edu/dce/computer-accounts.html
    2. Open a CatNet Account.
  2. Register for NURS 309.
    1. http://www.linfield.edu/dce/online-registration.html
  3. Criminal Background Check
    1. Order a background check through www.certifiedbackground.com
    2. Click on "Students"
    3. Enter the following in the Package Code box: IF78.
    4. Save a copy in your files, and send a copy to address below.
  4. Health Passport Form: A completed Health Passport signed by NLRC staff and your clinical instructor is required to participate in clinical experiences
    1. Note: Requirements that expire during the upcoming term must be updated before the term starts.  Submit updated documents to Joyce Betita, RN, MSN/Ed, NLRC Director, at jbetita@linfield.edu or the NRLC Staff at NLRCstaff@linfield.edu to update the passport.
    2. Health requirements: click here for a tool to track acquisition of required documents.  NOTE: This form is not documentation; you must have official provider, lab, or certification documents to verify your immunizations/immunity, CPR, etc.
    3. All students are required to complete the StudentMax Connection Orientation/CEU:  
      1. Go to www.linfield.edu, then Portland campus
      2. Click on Quicklinks, More
      3. Click on Nursing Learning Resource Center
      4. Scroll down to Health Passports, Passport and Site Specific requirements and click on that link.  This page has links to training modules for orientation and requirements for various clinical sites.  There are specific links to instructions for RN-BSN Online Students for the StudentMax orientation
      5. For annual core training modules, click on the StudentMax Connection orientation link under General
      6. Scroll down to GO TO ORIENTATION and click there.  The User ID is clinical and the password is learn9 
      7. Scroll down to Healthcare and Graduate Orientation and click there.  Complete the quiz as directed in the instructions links 
    4. Check the Site-Specific Requirements for the clinical site you will be assigned to.  Complete those requirements and submit to your instructor for her/his signature on the passport.

      4. Scroll down to Nursing Lab Documents and click on that link.  This page has links to training modules for orientation and requirements for various clinical sites
      5. Scroll down to the clinical site you are assigned to and follow directions listed for that site.  If your clinical site is not listed, contact Debra Henry, RN, MSN, Clinical Facilities Administrator, at dhenry@linfield.edu for information on your site.
      6. Complete these requirements and submit any paperwork to your clinical instructor
