RN to BSN Health Passport documents

Welcome to the Online RN to BSN degree program of Linfield College and the Linfield-Good Samaritan School of Nursing.

As you prepare to join the RN – BSN cohort with whom you will take the Linfield nursing courses, we want you to be aware of the documents that are required by the Linfield-Good Samaritan School of Nursing to be received prior to registration for nursing courses.

When admitted to the Nursing major and before registering for NURS 309, Transition to Professional Nursing, please complete the following:

  1. Criminal Background Check
    1. Order a background check through www.certifiedbackground.com; use package code LI11. Instructions are linked here. You do not need to send a copy of the results to the college. Also available is an electronic storage for Linfield Students' medical records for a nominal fee with lifetime accessibility so documents will always be securely stored and accessible. Other services include reminders when a booster or renewal is due. Order a Medical Document Manager/Immunization Tracker through www.certifiedbackground.com; use package code LI11im.

      Note: You do not need to send a copy of the results to the college as authorized staff have access to the results online.

  2. Essential Functions Form
  3. Proof of health insurance, or annual purchase of such insurance through the college.
  4. Proof of a current unencumbered registered nurse license in the state where clinical experiences will occur.

A completed Health Passport signed by ELC staff and your clinical instructor is required to participate in clinical experiences (NURS 475).

Note: Requirements that expire during the upcoming term must be updated before the term starts.  Submit updated documents to Alex Asbury by email (aasbury@linfield.edu) or fax (503-413-7280), to update the passport.

  3. Completion of StudentMax Orientation:
    1. Go to www.linfield.edu, Nursing and Health Sciences
    2. Click on Quicklinks, More
    3. Under Experiential Learning Center, click on Passport and Site-Specific requirements. This page has links to training modules for orientation and requirements for various clinical sites. There are specific links to instructions for RN-BSN Online Students for the StudentMax orientation
    4. For annual core training modules, click on the StudentMax orientation link under General
    5. Scroll down to GO TO ORIENTATION and click there. The User ID is orientsmc and the password is practice
    6. Click on Healthcare and Graduate Orientation
    7. Review the modules and complete the quiz as directed in the instructions links
    8. Save a copy and email/fax your Certificate of Completion to Alex

      Please note: Do not complete StudentMax Step 3

  4. Completed site-specific requirements, if applicable (by the start of the assigned clinical)
