Enroll at Linfield | Register for Housing | Accept Financial Aid | | Set up my CatNet ID (Linfield email) and WebAdvisor Account | Reserve Fall Classes | Check my Linfield Email | Sign up for a Meal Plan | Submit an Immunization Form

 

How do I Submit an Immunization form?

All Linfield students must show proof they have received two doses of measles vaccine.

  1. Download the Immunization Form
  2. Return it to the Student Services office
